01Overview

The foot and ankle are among the most frequently fractured structures in the body, accounting for a significant proportion of all musculoskeletal injuries seen in both emergency and outpatient settings. The foot's 26 bones - along with the ankle's three-joint complex - are subjected to substantial forces during weight-bearing, sports, and daily movement, making them vulnerable to fracture from both high-energy trauma (falls, motor vehicle accidents, athletic collisions) and repetitive low-level stress. Fractures range from hairline stress fractures that are barely visible on initial X-ray to complex, comminuted injuries involving multiple bones, displaced fragments, and surrounding soft tissue damage. Prompt and accurate diagnosis followed by evidence-based management is essential to restore normal alignment, protect the healing bone, prevent complications, and minimize long-term functional impairment.

02Causes & Etiology

Foot and ankle fractures arise through two broad mechanisms. Acute traumatic fractures result from a single high-force event: a misstep that causes the ankle to roll (inversion or eversion injury), a fall from height with axial loading of the heel (calcaneus fracture), direct impact in a sports collision, or a crush injury. The pattern of the fracture - its location, orientation, and degree of displacement - reflects the direction and magnitude of the force applied. Stress fractures, by contrast, develop insidiously through repetitive sub-maximal loading that outpaces the bone's capacity for remodeling; the metatarsals (particularly the second and third) and the navicular are classic sites. In patients with osteoporosis or other metabolic bone diseases, fractures may occur with surprisingly little force. Certain anatomic locations - the fifth metatarsal base (Jones fracture zone), the posterior talar process, and the lateral process of the talus - are notorious for being missed on initial evaluation and carry significant risks if not managed correctly.

  • High-impact sports (running, basketball, soccer, gymnastics)
  • Sudden increase in training volume or intensity (stress fractures)
  • Osteoporosis or low bone density
  • Prior ankle sprains with residual ligament laxity
  • Obesity (increased mechanical load during falls)
  • Metabolic bone disease (vitamin D deficiency, hyperparathyroidism)
  • Ill-fitting or inadequate footwear
  • Occupations involving heavy lifting, heights, or machinery

03Signs & Symptoms

  • Sudden, sharp pain at the time of injury followed by persistent aching
  • Localized tenderness to touch over the fracture site
  • Rapid swelling and bruising around the foot or ankle
  • Visible deformity or abnormal position of the foot or ankle (displaced fractures)
  • Inability or significant difficulty bearing weight
  • A popping or cracking sound heard at the moment of injury
  • Stress fractures: gradual onset of activity-related pain that improves with rest and worsens over weeks if untreated
